What to Bring to Your Appointment

• The patient's insurance card
• Physician referral forms if required by the insurance provider
• A list of current prescriptions and/or over-the-counter medications, including the dose and frequency
• Pertinent information about the patient's medical and surgical history

Your First Visit

At your first visit, the following forms will be printed for you. Please review the forms so that you have the necessary information with you to complete them at the office.

Insurance and Payment Information

Southwestern Institute of Neurology is a provider for Medicare and most major insurance plans. We provide insurance billing. Anything not covered by insurance will be the responsibility of the patient or guardian.

We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may also require you to provide a co-payment at the time of your appointment. When necessary, our staff will work with patients who require a payment plan.
